Swift received the inaugural Artist Director Honors, an award recognizing artists whose work behind the camera has expanded music videos and visual storytelling. The honor moved her ahead of Beyoncé, with whom she had previously been tied at 30 VMA wins.

Swift said she has been involved in around 60 music videos throughout her career, including 18 that she wrote and directed. She also won Video of the Year for “The Fate of Ophelia,” from her album The Life of a Showgirl. She dedicated the award to Dolly Parton, who died in August at the age of 80.

During the ceremony, Swift premiered the video for “Patient Zero,” which she directed. The production stars Colin Farrell and Dakota Johnson, who portray mirror images of each other. The song is Swift's first single since her July marriage to Kansas City Chiefs tight end Travis Kelce.

Madonna opened the ceremony with Sabrina Carpenter, performing “Bring Your Love.” The pair later won Best Collaboration for the song, while Madonna also received Artist of the Year and Best Dance.
